Rates and Insurance Information

 

The details about the cost of services.  Please read this part carefully.

*Note:  At this time, I am not accepting new patients with United Behavioral Health or Optum.

*AS WITH ANY INSURANCE PLAN, please check your coverage carefully by asking your insurance provider the following questions:

  • What kind of mental health coverage do I have?
  • Do I need a referral or authorization from my primary care provider?
  • What is my deductible?  Has it been met at this time?  What is my co-pay?
  • What is the coverage amount per therapy session?
  • How many sessions am I allotted per year?

Rates
Private pay sessions are $180 per clinical hour.  Initial assessment (intake) appointments are $250.  Insurance-based “customary rates” will be accepted for specific services.

Reduced Fee

Reduced fee, out-of-pocket services are available on a limited basis.  These discounts may NOT be applied with insurance billing.  Please ask before you receive services for more details.

Services already rendered may not be subject to a reduced fee.

Payment
Payment is expected at the beginning of each session.  Cash, check, or credit cards are accepted for payment.  For your convenience, you may also pay in advance using PayPal:  http://paypal.me/ProspicarePLLC

You will receive a bill for unpaid services.  If you have an out-of-pocket balance owing over $300, your treatment may be postponed until payment is made.  We will discuss your options if this is the case, as your care is a primary concern.

Cancellation Policies – please read carefully.
Please understand that your appointment time is reserved for you.  Your regular participation in therapy is a major factor in your healing and recovery.

If you do not come for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you may be required to pay the full cost of the session (unless due to an illness or emergency).  A bill will be sent directly to you for the full fee.

Please note:  because of the high demand for certain hours (e.g. late afternoon), “preferred” appointment times (consistently scheduled for you, weeks at a time) will only be offered to patients who maintain attendance according to these guidelines.  Thank you for understanding.
